terms and conditions before submitting

Hi All, im currently running the latest version of HESK and i have about 98% of it working how i want it.
the way i am using it is for multiple clients (amongst the clients we have contracted support and a pay as you go support)
i am looking for a sort of 'by submitting a ticket you agree to the payment scheme as below (or previously agreed)
i created a custom field as a check box option (made it mandatory) and added it to the ticket form, however i would prefer it was before. ideally i would like it after clicking the submit ticket button and before the category choice.
i would like to mention i have no clue about php but if someone can help me in the right direction i would like to think i can figure it out.
