LDAP - AD search

I would like to add in the main page of creating a ticket some fields to fill-in automatically NAME and EMAIL directly from a AD/LDAP search.
Of course, this could be an option in parameters of the application.

I started but i'm not so experimented :
Field One = a text box with NAME of people searched.
Field Two = a button that search in AD/LDAP names beginning by what you entered in field ONE
Field Three = a combox populated by Name/Email found

When you select a name in the ComboBox, its fills automatically fields NAME and EMAIL (satndards fields).

Any luck on this? I'd like to integrate Hesk with LDAP. I'd prefer to force users to login to use hesk and then it auto fill some blanks like user name and email.
