Only four standard replies show up

In our hesk installation we have about 5 standard replies created, but when we want to choose a standard reply on a task, only four replies show up.

So my list at admin/manage_canned.php shows 5 standard replies, but on a task (admin/admin_ticket.php) only gives me 4 options. The one at the bottom is missing.

Is this a bug or just me doing it wrong?

What version are you using? If not the latest, please try upgrading first.

If you are using the latest version, are you able to give me access to the help desk to have a look (not here, in a private message)?
