Clearing form fields problem!

It really is an excellent application. Thank you to everyone who contributed. I will use licensed use to support HESK developers.

I am facing a situation that I cannot solve. I need help with this.

I fill in the relevant fields to create a new request. (Mail notification, management, reply) Everything is so good so far.

When I want to open a new request on the same device (computer or mobile device), the fields I wrote before are filled. Idont want this. When I create a new request on the same device, the previous information will not come. All form entries are blank. How can I do that?
