How to add "Was this artcile helpful" Yes/No

I see other support page's bottom has those line

"Was this article helpful? Yes / No, I want to contact support"

I see => setting=>knowlegebase=>Enable KB rating => YES

but at the bottom of each article, I don't see the rating...am I on the right track? pls help

The rating is only visible from the public knowledgebase (http://www.slabburgers.com/support/knowledgebase.php), not the private (from staff pages) one.

Try adding a public article, opening it from the public knowledgebase pages and you should see it.
