I'm out of the country and will look at it over the weekend.

The problem is I can't reproduce most of these problems on my test setup, it is possible a server-side setting is affecting this.

If you would be by any chance prepared to give me access to your server (with a dedicated FTP account with access just to the help desk folder) so I can run some tests you are welcome to send me details over PM.
