Have set php max_execution_time=300 and added fastcgi_read_timeout 300; to nginx.conf but still get errors.

What am I missing?

Something is obviously timing out the script execution. I'm afraid I don't have experience with Nginx, so you will need to go through server logs/settings to locate and troubleshoot the issue.
