Adding an additional field to the request creation form

Good afternoon. Can you please tell me how to add an additional field for user to enter a phone number when creating a request with the output of this field in the future when displaying requests to the administrator. Thanks in advance for your help!

Login into your admin panel > Tools > Custom fields. There you will be able to create custom fields that will be presented when creating a ticket.
