You cannot integrate the Hesk form into other parts of your website.
However, you can:
1. Modify the look of Hesk a bit:

2. Or you could create a custom form, send results to an email account and convert emails into tickets (email piping or POP3/IMAP fetching). Seems like an overkill though.
