Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

You are probably using an older version of Gbook? It contains some encoded code that some server security scanners detect as "obfuscated PHP code" and delete the file, even though it doesn't do anything harmful.

To fix upgrade your guestbook to the latest stable version 1.8.2
