Autofill EmailId and Name

I have my staff working on HELP Desk
My issue is as follows:

If the staff click on New Ticket

The name and email fields in the "Insert a new ticket" form should be autofilled the respective username and email-id and this should be disabled for editing.

Please help me on how this would be done

Is this the new ticket page accessed through the admin panel or on the default home page?

Since subairmk says the above, I assume it's in the admin panel. It may be a misunderstanding.

If the staff click on New ticket, that name and e-mail field on the /admin/new_ticket.php page are meant to be the client/customer so there would be no way to auto populate that field. The name and address field is not for the hesk USER/Staff, that is what the "Owner: Assign this ticket to" drop down box is for.
