I would like to add a disclaimer to the blank ticket (submit a ticket page). what file would I modify to add it just below the "Message" field.

You can change the 'Before submitting please ensure...' text by editing language/en/text.php

Note that to use this you will also need to enable "Submit notice: [?] Show notice to customers submitting tickets" setting in HESK (admin panel > Settings > Misc tab)
