Came here to ask this.
I also followed the instructions, searched google, searched this form, then did it again

CC function is pretty important, lots of customer email in from name@domain, cc: othername@domain and both participants want to have access and/or contribute.

also, when I reply using the form only the first email is notified with updates (i.e. name@domain), not any further emails (i.e. othername@domain)
