I'm saying it's not very likely, but it wouldn't be the first time I've seen it, things like "mechanical turks" are more and more common and affordable.
Do you have *email piping* or *pop3 fetching* enabled? This is another source of how tickets are submitted.

You know I to have to put my two-bits in hey I beat you all with 128 and counting. First I thought it was my old script when this started on Oct 16 so I downloaded the new script and found my old script licence was no longer valid bought a new licence and still getting the same spam tickets. At one point it was every 15 minutes. These sites have there contact us blocked and i have tried through its server with no luck. Waiting for a fix.

Have you tried as suggested in this thread?
1. Disable anti-SPAM image
2. Enable anti-SPAM question and write a *unique* question for your website
3. Do you have email piping or POP3 fetching enabled?
There are also several IPs listed in the topic, try blocking them from your server as a temporary solution.

Any progress on this? I have followed your 1. 2. & 3. as well as running the latest version, 2.4.1.
I am still getting peppered with spam messages daily. I have been adding the IP from the spam tickets to my IP Deny list as they come in, but I can't keep ahead of it. This is getting ridiculous.
I think it's safe to say at this point that there is a hole somewhere. It appears that the spam is still happening to the people who have done your item lists 1,2 & 3 as well as having the latest version, 2.4.1 installed.

Many have reported a stop of SPAM following these steps. From my logs it appears they are reading the captcha.
Care to share your help desk URL so I can check some things (can send it over a private message)?

P.s.: if you are still using the mathematical equation you posted earlier as the anti-spam question try using something non-mathematical (something in the sense of "are tomatoes an animal or a vegetable"). It's quite easy to detect and solve mathematical problems automatically.

Update: I just made ReCaptcha plugin available for HESK, see here:
HESK 2.4.1 ReCaptcha plugin
Anyone having problems with SPAM (not coming through email piping or POP3 fetching) should also give this a try.
*** If you try ReCaptcha plugin please provide feedback whether it was effective or not ***
