I want to modify the "default" SIGN Guestbook Form. I want to ask three questions. I figured out how to change the text for the first two default questions but I can't figure out how to allow the third default question (E-Mail) to simply be a text field. I would prefer to keep the E-Mail field so if someone could let me know how to add another text field - that would be great. I got all hung on on the T## stuff, etc.

Anyway, if someone could let me know that files to change, etc. - it would be appreciated.
